Practice Policies

Thank you for choosing Reflections Integrative Health for your health care needs. In an effort to make your transition to our practice as smooth as possible, we have the following policies that we request you read and sign annually. We are available to help clarify any of our policies. We strive to provide clients with the highest level of customer service and healthcare.  Our entire team appreciates and welcomes your feedback to improve services, address any personal concerns regarding your medical care, or office experience.

Office Hours

Office Hours are by appointment only. We are closed on all major holidays. Extended or after hours appointments may be available on a limited basis. Please call 911 in the event of any life threatenting emergencies.

Messages

Messages are returned as soon as possible. Non-urgent calls and messages will be returned within 48 hours. Please call the office directly for any urgent concerns. Conact 911 or go to the emergency room for any emergencies or life threatening conditions.

First Visit

New clients are welcome. It is recommended that you complete and submit the intake and all other forms sent to you via email 1-3 days prior to your appointment time. Failure to submit these forms prior to your appointment may result in rescheduling of your appointment.

Appointments

We value your time and want to give you and your health our utmost attention. If you arrive more than 15 minutes late for your appointment, you may be asked to reschedule in order for you to have ample time to have your health concerns addressed. Please provide 48 hours notice when canceling or rescheduling your appointment. A $50.00 fee will be charged for missed appointments, appointments canceled or rescheduled within 24 hours of the appointment time. By failing to cancel or reschedule your appointment, we reserve the right to dismiss the client from the practice.

We currently do not offer same day appointments or walk-in appointments. Contacting the office early in the day will help us accomodate your needs. We will make every effort to see you during your scheduled appointment time. We ask for your understanding in the event we are running behind schedule as unforeseen emergencies and complex patients may require additional provider time. Our staff is committed to keeping you informed of delays and offering our patients options to manage their valuable time.

Controlled Substances

Chronic pain management services with controlled substances or narcotics are not provided. Any chronic pain needs or other medical conditions requiring long term controlled substance treatement will be referred to provider who can better managem your chornic pain needs.

Medical Information and Records

To protect your privacy, employees are requested to seek secondary identification from all clients in person or over the phone. We ask for your cooperation in the verification process.

Client Dismissal

We sincerely hope that we never have to part ways with a client. However, extenuateing circumstances may make this necessary. IF this occurs, you will be notified by certified mail on this non-negotiable decision.

Financial Arrangements & Insurance

We are committed to providing you with the best possible care. This is a cash-pay only office. Insurance is not accepted for services offered in the office. If you have medical insurance, we would like to help you receive your maximum allowable benefits for services we provide; as well as independent services related to prescriptions, laboratory testing, diagnostic imaging, health promotion services and referrals. You will be provided with a superbill that can be submitted to your insurance for reimbursement.

Reflections Integrative Health is not responsible for reimbursment provided by insurance companies. Please be famillar with the terms and policies of your insurance plan. Any questions or problems with your insurance should be directed to your individual insurance company. Payment is due at the time services are renered. Cash, credit card and debit card transactions are accepted. We are unable to accept checks. There will be a $50.00 fee changed on all returned payments.

Prescription Refill Policy for Medications

We have zero-tolerance for errors related to patient prescriptions and medications. Please read the following prescription refill policies carefully:

1) Clients must speak directly to a memeber of the office staff during normal business hours.

2) Refills should be requested Monday – Thursday and that refills requersted after 4:00pm on Thursday or anytime on Friday may not be processed untill Monday.

3) A follow-up visit may be required in order to refill prescriptions.

4) For safety reasons, the office does not accept auto-generated refill requests from pharmacies as doses or medications are often incorrect or discontinued.

5) All medications are to be taken as instructed and are not to be altered or changed in dosage without consulting with a medical provider.

6) All follow-up appointments are to be attended as recommended so that medications and any relevant lab work can be monitored.

7) Altering or forging a prescription is a felony and will be reported.

8) Medications are not to be traded, sold or given away.

9) Only emergency medications will be called in after hours and will incur a $100.00 fee.

Laboratory Information & Policies

We do not provide testing in house at this time. It is your responsibility to be aware of your insurance coverage, policy provisions, exclusions, limitations and authorization requirements. Many insurance companies will not pay for “routine” lab tests and will requrie a medical diagnosis for each and every test that is done. In general, questions regarding bills you recieve from laboratory facilities should be directed towards the lab and/or your insurance carrier. Reflections Integrative Health has no control over this.

Email & Text Messaging

Email and/or text can be used to relay important information. Clients may be contacted via email and/or text message to remind you of appointments, provide general health reminders & convey information regarding test such as lab or imaging results.
